We are pleased to make the transcript request form available online.
1) Download the request form and save it onto your computer.
2) Open the form using Adobe Acrobat Reader. This form has interactive fields and you will be able to complete the form electronically.
3a) Sign the form using a digital signature, if you have one available. You may then submit the form via email using the "Submit via email" button on the bottom of the form. Your email client will open and opt to send the document as a PDF.
3b) Once the form has been completed, print the form, sign it and mail to the address shown on the cover page. You may also fax the request.
